Point-of-care infrared thermal imaging for differentiating venomous snakebites from non-venomous and dry bites

Background Local envenomation following snakebites is accompanied by thermal changes, which could be visualized using infrared imaging. We explored whether imaging used to differentiate venomous from non-venomous and dry bites. Methods prospectively enrolled adult patients with a history of snakebite in the past 24 hours presenting emergency teaching hospital southern India. A standardized clinical evaluation for symptoms signs including 20-minute whole-blood clotting test prothrombin time was performed assess status. Infrared done at enrolment, 6 hours, later smartphone-based device under ambient conditions. Processed images were independently interpreted twice reference rater once three novice raters. Findings studied 89 patients; 60 (67%) them male. Median (IQR) bite enrolment 11 (6.5–15) hours; 21 (24%) within snakebite. In all, 48 had local with/without systemic envenomation, 35 classified as non-venomous/dry Envenomation status unclear six patients. At area increased temperature around site (Hot spot) evident on 45 while hot spot only without envenomation. Presence baseline sensitivity 93.7% (95% CI 82.8% 98.7%) specificity 82.9% (66.3% 93.4%) envenomed those Interrater agreement identifying spots more than substantial (Kappa statistic >0.85), intrarater almost perfect = 0.93). Paradoxical changes observed 14 Conclusions Point-of-care useful early identification snakebites.
